Makeup model appointments may occasionally be offered on Fridays & Saturdays subject to availability. I confirm that I can make myself available for the appointments I apply for.*

I acknowledge that for lash & brow treatments a patch test is required 24–48 hours before my first treatment. Patch tests are available Monday, Friday, and Sunday between 18:30–20:30, and I confirm I will make myself available to attend.*

For lash treatments I confirm I will attend my appointment WITHOUT existing lash extensions.*

I understand that my appointment may be filmed or photographed and that all images and videos may be used by Fox & Doe, with no ownership or usage claims on my part.*

I understand that model appointments require a £20 fee, payable in advance to secure my slot.*

I understand this fee is non-refundable and applies to a discounted, content-based service.*

I confirm that I will attend my appointment well-presented, with hair done and suitable for filming/photography, and without eye makeup.*
